About the Community Health Nurse role

Join a passionate, multidisciplinary team delivering person-centred nursing care to priority groups in the community. Support clients with complex needs through assessment, care planning, health coaching and partnerships that promote equity, wellness, and self-management across all settings. Reporting to the Dietetics, Diabetes Education & Community Nursing Coordinator, this is a fixed term part-time role (0.5 FTE). Offices located in the Northern Suburbs.

  • Deliver expert, person-centred care across varied community settings
  • Collaborate to develop care plans that support client goals and needs
  • Track progress and use outcomes to enhance client health journeys
  • Lead innovative, evidence-based care and health promotion programs
  • Contribute to team growth through service design and evaluation

To succeed in this role you will have:

  • A qualification as a Registered Nurse Division 1
  • Registration with AHPRA as a registered nurse
  • Current Victorian Drivers Licence.
  • Smoking Cessation Worker trained (desirable).​​​​​​​
  • At least 3 years clinical nursing experience in a community setting with well-developed comprehensive complex care assessment, care plan development and care management skills.
