Job Description

About the Youth and Community Worker role

Join our Community Programs team to support young people (12-25 years) who are homeless or at risk. You'll help young people navigate health and wellbeing services and work directly within outreach services. By building strong community connections, you'll strengthen referral pathways and improve health outcomes for young people, particularly those in priority groups. Reporting to the Community Programs Manager, this is a ongoing part-time role (0.4 FTE). Offices located in the Northern Suburbs. 

Your Passion and Commitment will see you:

  • Provide individual and group support to at-risk young people
  • Connect young people to health and wellbeing services
  • Build community capacity to improve youth health outcomes
  • Develop and review program plans, workplans and budgets
  • Strengthen referral pathways for priority youth populations

To succeed in this role you will have:

  • Qualification in Social Work, Youth Work or a related field (essential). Additional qualifications in family-inclusive practice, alcohol and other drugs (AOD) or dual diagnosis (desirable).
  • Current First Aid and CPR certification (desirable)
  • Experience supporting young people at risk of homelessness
  • Experience applying community development with priority groups
  • Proven ability to manage projects from planning to evaluation
