About the Registered Nurse (Care Coordinator) role

Join YourCH as a Care Coordination RN delivering the Seniors Lifestyle Victoria service, funded by the Department of Health. Support older Victorians with mild to moderate frailty, helping them manage their health, build independence and improve wellbeing through coordinated, person-centred care. Reporting to the Clinical Nurse Coordinator, this is a part-time role (0.8FTE), fixed term until 30 June 2028. Offices located in the Northern Suburbs. 

Your Passion and Commitment will see you:

  • Assess client health and provide advice on managing conditions.
  • Empower clients to set goals and create personalised care plans.
  • Identify gaps in services and connect at-risk clients to support.
  • Facilitate referrals and guide clients through intake pathways.
  • Collaborate with GPs, nurses, and Allied Health for integrated care.

To succeed in this role you will have:

  • Bachelor of Nursing, or equivalent.
  • Registered Nurse, Division 1 with current and unrestricted AHPRA registration.
  • Current Victorian Drivers Licence.
  • Minimum 5 years experience in Community Health sector, or equivalent.
  • Experience in providing complex care coordination and carrying a client caseload required. Experience in HITH, HARP or similar highly regarded.
