About the Aged Care Assessment Officer (Comprehensive) role
Join our Aged Care Assessment Team and help improve access to care for older people (65+) across Darebin, Yarra, Melbourne and Maribyrnong. You’ll deliver timely, person-centred assessments, including home visits that support independence, wellbeing and informed choice. Multiple fixed-term part-time and full-time opportunities available. Offices located in the Northern Suburbs.
Your Passion and Commitment will see you:
- Conduct comprehensive client assessments (home, face-to-face, phone) via My Aged Care, aligned to client needs and goals
- Partner with clients to set goals, build support plans and coordinate referrals to support independence at home and in community
- Manage diverse caseload, including dementia, CALD, LGBTIQA+ and Aboriginal and Torres Strait Islander clients
- Promote client choice and shared decision-making in care planning
- Prioritise and manage workload effectively to meet service demands and objectives
To succeed in this role you will have:
- Relevant tertiary qualification in Allied Health, Nursing, Social Work or a related health discipline.
- Current unrestricted registration, or eligibility for registration, with AHPRA or a relevant professional body.
- Minimum one year’s experience delivering services to older people in aged care or community settings, including dementia care and working with diverse communities.
- Experience supporting people from culturally and linguistically diverse backgrounds.
- Sound knowledge of the My Aged Care Portal and Integrated Assessment Tool (IAT).
