About the Community Health Nurse - General Practice role

Reporting to the Clinical Nurse Coordinator, this is a casual position based in the Northern Suburbs.
Your Passion and Commitment will see you:
- Deliver safe, evidence-based care that supports the health and wellbeing of our community.
- Provide a broad range of nursing care, including triage, immunisations, wound care, ECGs, health assessments and care plans.
- Work closely with GPs to support clinical procedures and deliver high-quality patient care.
- Coordinate care with GPs and the wider team to achieve the best outcomes for patients.
- Support people living with chronic conditions through proactive, person-centred care.
To succeed in this role you will have:
- Relevant nursing qualification.
- Current Division 1 Registered Nurse with AHPRA registration.
- Experience working in a General Practice setting.
- Strong understanding of primary care, community health and the broader health sector.
- Experience using clinical software, recall and reminder systems, and clinical data management.
