About the Community Health Nurse - General Practice role

Reporting to the Clinical Nurse Coordinator, this is a casual position based in the Northern Suburbs.

Your Passion and Commitment will see you:

• Deliver safe, evidence-based care that supports the health and wellbeing of our community.
• Provide a broad range of nursing care, including triage, immunisations, wound care, ECGs, health assessments and care plans.
• Work closely with GPs to support clinical procedures and deliver high-quality patient care.
• Coordinate care with GPs and the wider team to achieve the best outcomes for patients.
• Support people living with chronic conditions through proactive, person-centred care.

To succeed in this role you will have:

• Relevant nursing qualification.
• Current Division 1 Registered Nurse with AHPRA registration.
• Experience working in a General Practice setting.
• Strong understanding of primary care, community health and the broader health sector.
• Experience using clinical software, recall and reminder systems, and clinical data management.
