About the Aged Care Assessment Officer (Comprehensive) role
We are seeking Clinical Assessors with recent ACAS/RAS assessment experience and tertiary qualification in Allied Health, Nursing or Social Work to:
- Conduct comprehensive care needs assessments for individuals aged 65+
- Develop and review care plans to support independence and wellbeing
To be considered for this role, you will have:
- Relevant tertiary qualifications and current professional registration (if applicable).
- At least one year of experience conducting ACAS/RAS clinical assessments.
- Proficiency in using the My Aged Care Portal and a solid understanding of the Integrated Assessment Tool (IAT).
- A demonstrated commitment to ongoing professional development and evidence-based practice.
- Exceptional relationship-building skills to effectively engage with clients and empower them in their healthcare journey.
- Current Driver's License - travel required
