About the Community Health Nurse (General Practice) role
Join a dynamic, collaborative team as a Community Health Nurse, where you’ll play a key role in shaping the health and well-being of our community. Take charge of clinical duties, design patient care plans, and build strong partnerships within a multidisciplinary team. Work alongside GPs, nurses, and external services to deliver high-quality primary care, driving better health outcomes for our community.
Reporting to the Clinical Nurse Coordinator, this is a 0.6FTE fixed-term role until 30 June 2025.
Your Passion and Commitment will see you:
- Deliver clinical care, including triage, immunisations, wound care, and chronic disease management.
- Coordinate care plans, health assessments, and health education programs.
- Work closely with a multidisciplinary team, ensuring holistic patient care.
- Provide person-centred care, collaborating with patients, families, and service providers.
- Respond effectively to changing situations and escalate issues as required.
To succeed in this role you will have:
- AHPRA Registration as a Registered Nurse (Division 1) and Nurse Immuniser (highly desirable)
- A strong background in a General Practice setting with a solid understanding of its connection to the broader health sector and community health priorities
- Outstanding interpersonal, communication, and problem-solving skills
- Extensive experience with recall and reminder systems, data, and clinical software systems
- Strong organisational skills with the ability to prioritise tasks effectively
